Leadership Awards

Thresholds
Thresholds is a community mental health center that provides healthcare and housing for thousands of people with mental Illnesses and substance use disorders in Illinois each year. Thresholds first used telehealth in a pilot to provide psychiatric care for people who were frequent users of hospital emergency departments and inpatient psychiatric services. Thresholds partnered with a Medicaid Managed Care Organization. Thresholds staff would go to the home of a patient and connect them on their laptops to a Thresholds psychiatrist.

Thresholds currently uses telehealth to provide telepsychiatry services in conjunction with two Assertive Community Treatment (ACT) teams located about 60 miles from Chicago. The psychiatrist participates in weekly meeting of the ACT teams and provides services to clients in their homes or in a nearby Threshold office. Thresholds also uses telehealth in its Substance Treatment Program, allowing a physician to evaluate and treat persons with Opioid Use Disorder in remote locations. Since the beginning of this program three years ago, Thresholds has prescribed potentially life-saving medications to approximately 40 individuals.

 Jesse Brown VA Medical Center
The Jesse Brown VA Medical Center consists of a 200-bed acute care facility and four community-based outpatient clinics (CBOCs). Jesse Brown VAMC provides care to approximately 62,000 enrolled veterans who reside in the City of Chicago and Cook County, Illinois, and in four counties in northwestern Indiana. Jesse Brown has been named the Telehealth Hub for the Veterans Integrated Service Network 12. The Hub coordinates telehealth care of 7 hospitals in Illinois and Wisconsin.  This year, Jesse Brown coordinated telehealth care to over 8,000 veterans, providing care in 12 different specialties. Jesse Brown has stories from veterans praising the quality of telehealth consultations, which avoid their having to travel hours to a VA site.

Illini is working with other Illinois Telehealth Network members to create a heroin and opioid treatment and education network model through telemedicine. Illini provides 1,500 telehealth visits a year.

Southern Illinois Healthcare
Carbondale-based Southern Illinois Healthcare (SIH) is a nonprofit health system spanning the southern third of Illinois. SIH currently operates telehealth inpatient and outpatient programs for stroke, behavioral health and cardiovascular services.  It also offers Maternal Fetal Medicine services and prompt/urgent care e-visits on an outpatient basis. Recently, the SIH system made a full commitment to integrating telemedicine into its provision of service model by establishing a system-wide telemedicine division.
